Position Overview :
The Outreach Specialist will coordinate client outreach efforts for Health Homes programs throughout Staten Island. Key responsibilities include conducting home visits to identify and enroll eligible members, re-engaging lost clients, collaborating with community providers, documenting encounters, and supporting operations.
Compensation : $22.00 / HR
Location : Staten Island, New York
Schedule : Monday - Friday, 9 : 00 am - 5 : 00 pm
Responsibilities :
149; Work with supervisor to identify outreach clients
149; Conduct home visits, phone calls, letters to enroll eligible members
149; Complete progress notes, screenings and assessments
149; Document all activities in program database
Qualifications :
149; High school diploma or equivalent
149; Experience with homeless / HIV groups preferred
149; Bilingual Spanish / English a plus
149; Valid NY drivers license with clean record
149; Strong communication and documentation skills
